场景一:钢构吊装临边防护验收 Steel Erection Leading Edge Protection Acceptance Steel erection supervisor, I'm here to sign off on the leading edge protection for the inverter station roof steel — Level 3, 12 meters. Decreto 911/96 requires guardrails at 1.1m height, mid-rail at 0.55m, and toe board at 150mm. Walk me through your installation.

Perimeter guardrail system installed on all four edges — total 78 linear meters. Top rail at 1.1m — (measures) 1.11m, within tolerance. Mid-rail at 0.55m. Toe board at 155mm — 5mm over spec but acceptable. All posts are clamped to the steel beam top flange with rated clamps — 150 kg lateral load capacity per post. No drilling, no welding on the structural steel. Access is via the stair tower on the east side — no ladders on the roof edge. The steel deck is not yet installed — right now it's open beams. Workers must stay on the designated walkways — those two 600mm-wide planks with handrails on both sides.

I'm measuring the guardrail load. (applies 90 kg lateral force at mid-span) No visible deflection — passes the load test. What about the gap between the guardrail and the building edge?

Maximum gap 150mm — per Decreto 911/96, the gap shall not exceed 200mm. We're within spec. The only openings are the two future skylight openings — both covered with rated covers, marked "HOLE — DO NOT REMOVE," and the covers are secured against displacement. Access to remove them is only permitted after the roof deck is fully installed and the skylight frames are ready for installation.

Who is your rescue person if someone falls while tied off?

We have two rescue-trained riggers on site. Retrieval equipment — a 15m rope with descent device — staged at the access point. Rescue drill was done last Tuesday — time to recover a suspended person: 4 minutes 20 seconds, within the 6-minute suspension trauma window. The rescue plan is posted here on the access board.

场景二:脚手架挂牌与停工 Scaffold Tagging & Stop Work
(on radio) HSE control, this is Miguel at the Zone D transformer platform scaffold. I just climbed up and three scaffold boards on the second lift are cracked — visible splits through the timber. The scaffold is red-tagged from yesterday's rain inspection, but someone removed the tag and put a green tag on it this morning. I'm not going up there.

(arriving) That was me. The planking subcontractor was supposed to replace the planks this morning at 07:00. They didn't show up. The electrical termination crew needs to be on this platform by 09:00 or we lose a day on the schedule. I thought we could use it for one shift — the cracks aren't all the way through.

Let me be absolutely clear. You removed a red "DO NOT USE" scaffold tag and replaced it with a green "SAFE TO USE" tag on a scaffold you knew had damaged planks. You sent workers onto a scaffold you knew was unsafe. That is a Level 1 safety violation — willful disregard of a known hazard. This scaffold carries a live electrical termination crew — it's not just a fall risk, it's an electrocution risk if a plank fails and someone grabs a live cable on the way down. The scaffold is red-tagged — again. The electrical termination is postponed until the planks are replaced and a competent scaffold inspector re-inspects and issues a new green tag. You — supervisor — are suspended from site for 3 days pending a formal investigation. You will complete an 8-hour scaffold safety retraining before you supervise any scaffold work again. I'm filing a formal incident report. This is exactly how people die on construction sites.
