场景一:逆变站中压开关柜LOTO程序 Inverter Station MV Switchgear LOTO Procedure We're isolating the MV switchgear CB-304 in Inverter Station F-04 for cable termination work. I've got the switching schedule approved and the electrical work permit issued. Authorized electrician: I need you to execute the LOTO sequence now.

Electrician: (reads switching schedule) Step 1: open CB-304 at the local control panel. (opens) Breaker open — position indicator confirms "OPEN." Step 3: close and lock the shutter over the busbar contacts. Shutter locked with lock #1. Phase B — 0V. Phase C — 0V. All three phases confirmed dead. Applying personal ground set: Phase A to ground — Phase B to ground — Phase C to ground — all three phases grounded. Ground cable connected to the station earth bar. Lock #A-47 key placed in the group lock box. The cable termination crew can now collect their personal locks from the lock box and add them. The circuit is isolated, locked out, tagged out, tested dead, and grounded. Safe for work. Handing over the isolation certificate to you.

场景二:光伏组串直流危险——不安全的组串测试 PV String DC Hazard — Unsafe String Testing
(arriving at Zone A PV array) Stop what you're doing. You're testing string open-circuit voltage with your multimeter probes directly on the MC4 connectors — in broad daylight, with the panels generating. That's 850V DC at up to 14 amps flowing through those connectors the moment you unplug them. You're not wearing arc-rated gloves, no arc-rated face shield, and you haven't de-energized the string. What procedure are you following?

"We do it all the time" is how people get killed by DC arcs. PV modules are always live in daylight — you cannot turn them off. DC arcs don't self-extinguish like AC arcs — they sustain at high temperatures and can reach 4,000°C. An MC4 connector separation under load creates a sustained DC arc — that's a fire ignition source directly underneath a glass panel field. From now on, the procedure for string testing is: (1) cover the modules with opaque blankets to de-energize the string — you test in the dark or under cover. (2) wear 8 cal/cm² minimum arc-rated PPE — gloves, face shield, long-sleeve shirt. (3) use insulated test probes rated for 1500V DC with fused leads. (4) never separate a DC connector under load. I'm issuing a safety stand-down for all commissioning technicians this afternoon — 2-hour DC safety retraining. And I want these strings re-tested using the correct procedure. No exceptions.
